Health Literacy and Chronic Disease Self-Management

Department of Family and Community Medicine

This study assesses how health literacy and other factors relate to control of Diabetes and blood pressure in people with more than one chronic problem. That is important because frequently people have more than one chronic condition.

Our goal is to determine the impact of health literacy on intermediate patient outcomes, self-care behaviors, and how literacy influences a patient’s perception of the health care environment. Among patients with diabetes and at least one other chronic condition, we will assess how poor health literacy is associated with intermediate outcomes (e.g., glycohemoglobin), explore the association between health literacy and self-care behaviors (e.g. diet, exercise, blood glucose testing, foot care, smoking), and assess how patients across the literacy spectrum perceive the health care environment.
